What is Support coordination ?

Support coordination helps people with complex and multiple disabilities to identify and access the support and services needed to reach their goals. Support coordination is an essential service that enables individuals with complex and multiple disabilities to access the necessary supports and services to achieve their goals. The Support Coordination team comprises of highly trained professionals, including social workers, case managers, and health experts. They provide customized co-ordination services that cater to the unique needs and objectives 

NDIS customers requiring childhood services through to youth and adults with various psychiatric, cognitive, physical, sensory, acquired, neurological, and/or psychosocial disabilities. Their expertise also extends to individuals with a high level of complexity in their lives, ensuring they receive the best possible care and support.

How can a Support Coordinator assist me?

Assistance with implementing your NDIS Plan is provided by Support Coordination, which is funded by the National Disability Insurance Agency (NDIA). It is recommended to discuss the required support with your planner when creating your plan. If you have other funding sources, such as TAC, WorkCover or Department of Veterans Affairs funding, we can help you coordinate your services as needed.
